This is a Failure Mode and Effects Analysis format in Microsoft Excel modified somewhat to provide some additional capability from its original shareware format. It's a good tool to organize your thoughts as you work on a design. I find it best to start using this as soon as a design is conceptualized and make notes as I go rather than wait until it is completed. That makes this utility a more effective tool and helps focus on reducing built in failure conditions and identifying ones that are unavoidable.
